After sending a form, message, portal request, referral, or documents, please also call (661) 776-3876 Monday–Friday, 8:30 a.m.–5:30 p.m. PST to follow up. Submission does not confirm, cancel, or reschedule an appointment. For urgent needs, call during office hours. For a medical emergency, call 911.
