Request OnPatient Portal Access
Select OnPatient portal access in the form and provide your email address.
- Request a portal invitation from our office and confirm the email address we have on file.
- Open the most recent invitation emailed to you and follow its activation link.
- Complete the identity details and create your account using information that matches your office record.
- After activation, visit OnPatient Portal Login and sign in normally.
We recommend activating within 24 hours of receiving your email. DrChrono states that invitations expire after 7 days, or when a replacement invitation is sent. If the link fails, ask our office for a new invitation.

We strive to review your request and respond by the next business day. After sending a form, message, portal request, referral, or documents, please also call (661) 776-3876 Monday–Friday, 8:30 a.m.–5:30 p.m. PST to follow up. Submission does not confirm, cancel, or reschedule an appointment. For urgent needs, call during office hours. For a medical emergency, call 911.
